Application Process
Prospective grantees must complete and submit a Preliminary Grant Inquiry form by July 21st. This form must be accompanied by a copy of the organization's IRS Tax Exempt status letter. Following an initial screening, certain applicants will be invited to submit Grant Applications, which are due by September 1st.
The following Preliminary Inquiry forms in PDF format are available for download. Preliminary Inquiries may be submitted by mail or electronically and must be received prior to 5 PM in our office on or before the deadline date, July 21st.
The following Grant Application forms in PDF format are available for download.
Grant Applications must be received by mail only, physically, in our offices
prior to 5 PM on or before September 1st.
Note: The following form cannot be submitted without prior approval.
