Grant Application Process

Criteria For Application Review
To be considered for funding, an organization must be designated as tax-exempt under Section 501(c)3 of the Internal Revenue Code. Grant Healthcare Foundation will not accept proposals from outside the Chicago metropolitan area. Multi-year commitments are rarely made, and usually only after a finding that a specific program will not go forward without such a commitment. Multi-year commitments are subject to annual review and timely receipt of all required reports.

Grant Healthcare Foundation is most interested in organizations that have:

  • Programs that directly provide health services and benefit a large number of persons
  • Responsible and measurable goals and objectives aimed at promoting and preserving health
  • Innovative model programs which can be readily adapted for use by other organizations
  • Consistent and effective management
  • A broad base of support

    Grant Healthcare Foundation will not make grants to support:

  • Individuals
  • Organizations that illegally discriminate on the basis of race, sex, creed, age or national origin
  • Religious purposes or religious organizations whose services are limited to any one sectarian group
  • Political activity or lobbying
  • Fund-raising events
  • Endowments
  • Indirect costs of grantee organizations
  • Costs of ADA compliance by non-healthcare providers

    Grant Application Process
    Prospective grantees must complete and submit a Preliminary Grant Inquiry form by July 21st. This form must be accompanied by a copy of the organization’s IRS Tax Exempt status letter. Following an initial screening, certain applicants will be invited to submit Grant Applications, which are due by September 1st. The following forms in PDF format are available for download:

  • Grant Application (do not submit without prior approval)

    Awarding Of Grants
    If a grant is approved, a payment and report schedule will be included in the grant agreement letter. Funds will be disbursed according to the schedule approved by the Foundation Board; however, most grants are paid in December of each year. The Foundation reserves the right for its representatives to conduct site visits to prospective grantees prior to the awarding of a grant, during the period the grant is being expended or after the grant period has ended.
